You are the one filing the appeal — UPHELD helps you do it correctly and quickly. We are not a firm, and we do not provide professional advice of any kind.
Which insurance plans does UPHELD work with?
At launch, UPHELD works with plans that use FHIR Patient Access APIs — including Medicare Advantage, Medicaid managed care, ACA Marketplace plans (through HealthCare.gov), and most fully-insured commercial plans from major carriers including UnitedHealthcare, Aetna, Cigna, Blue Cross Blue Shield, and Humana. Self-funded employer plans are on our roadmap but are not included at launch. If you are unsure whether your plan qualifies, enter your email and we will confirm when you sign up.
How much does it cost?
UPHELD charges a flat monthly fee. No percentage of anything. No contingency. No "we take a cut if you win." The fee covers unlimited appeal monitoring and letter generation for your active plan. Exact pricing will be announced with the product launch — waitlist members will get first access and a discounted rate.
Do you guarantee that my appeal will succeed?
No. No one can guarantee that. What we can tell you is that 44% of internal appeals filed on ACA Marketplace plans were overturned in 2023 (KFF), and 83.2% of prior authorization appeals in Medicare Advantage were partially or fully reversed in 2022 (AMA / KFF CMS data). Filing a well-constructed appeal is your best available move. UPHELD makes it possible to actually file one.
What happens to my health data?
Your data is used only to read your claims, identify denials, and generate your appeal letter. We do not sell it. We do not share it with insurers or third parties. UPHELD operates under a Business Associate Agreement (BAA) as required under HIPAA. You can revoke access at any time and request deletion of your data.
Insurers process hundreds of millions of claims per year. Nineteen percent of them get denied. Almost none are appealed. UPHELD is being built to change that — starting with the people who are paying for coverage and getting nothing back when they need it most.
